1. General

• Patients must provide accurate personal, insurance, and medical history information at the time of registration.

• Avani Hospital reserves the right to modify service charges, policies, or procedures without prior notice.

2. Appointments and Consultations

• Prior appointment booking is recommended; emergency walk-ins will be attended based on priority and doctor availability.

• Consultation fees are non-refundable once the service is rendered.

3. Admission and Discharge

• Admission is subject to room and bed availability.

• Discharge will be processed only after the final clearance of hospital bills and formalities.

• The hospital is not responsible for personal belongings lost or misplaced during the stay.

4. Payments and Billing

• All bills must be cleared as per the hospital’s billing cycle (advance deposits, interim payments, and final settlement).

• Payments can be made via cash, UPI, or approved insurance coverage.

• For insured patients, authorization from the insurance company is mandatory before treatment/surgery.

5. Medical Records and Privacy

• Patient confidentiality will be maintained in accordance with applicable laws.

• Copies of medical records can be provided upon request, subject to processing charges and hospital policy.

6. Patient Conduct

• Patients and their attendants are expected to maintain discipline and respect hospital staff and property.

• Any damage caused intentionally or negligently will be charged to the patient or attendant responsible.

7. Emergency Services

• Emergency services are provided based on medical urgency and triage protocols.

• Treatment in emergency cases is prioritized over financial formalities.

8. Use of Hospital Facilities

• Hospital facilities are meant strictly for registered patients and their authorized attendants.

• Unauthorized photography or video recording within the premises is strictly prohibited.

9. Rights Reserved

• Avani Hospital reserves the right to refuse treatment or discharge a patient under circumstances like non-compliance, abusive behavior, or safety concerns.

10. Legal Compliance

• All services are provided under the jurisdiction of Lucknow courts in case of any legal disputes.
